Accident reports show that as many as 90% of industrial accidents have been attributed to human error. Data shows 94% of car accidents are caused by Human Error. Medical Human Error in hospitals is the 3rd leading cause of death. The average company wastes $637 per year per employee on human errors. For energy industry, that number is much higher.
